Attention Medicare part D Retirees: You may be
receiving a phone call from Excellus (through the ELIZA Corporation) regarding
your prescription drug coverage. This is a Coordination of Benefits (COB)
Survey. This is information that (CMS) the Centers for Medicare and
Medicaid Services requires so that member claims are processed correctly.
Your
coverage will not be changed as a result of this survey. It only impacts
which payer is the primary payer if you have other coverage.
We
appreciate your cooperation.
HEALTH FUND: By now everyone should have received your new Summary Plan Description (SPD) in a 3-ring. If you have not received one please give the benefits office a call.
PERSONAL INFORMATION: If you have moved or have changed phone numbers or if you have changed your marital status, beneficiary status or dependant status please be sure to inform our office. We cannot help you if we cannot find you.
As
always, please call us in the Fund office with any questions or concerns you
may have.
