LOCAL #46 BENEFIT NEWS

HRA / ANNUITY SELECTION FORM: You will be receiving a selection form in the mail this week and you will need to make a decision as to how you would like to divide your $2.00 contribution. Once received please be sure to make a selection and return it to the benefits office ASAP. All forms must be received NO LATER THAN JUNE 26th, 2009.

NOTE: If you do not make a selection you will automatically be placed in the default option.

HOURS REPORT: If you have not received this new (1st quarter) hours report in the mail please let us know. This new statement is a summary of the hours reported to the benefits office, by your employers and you will be receiving one of these statements every quarter. These are the hours that benefits are paid upon on your behalf. Please keep in mind that when it comes to overtime, benefits are paid for hours worked, they are not paid at a premium rate. As always, keep your pay stubs and compare this report against your records on a regular basis, if you find any discrepancies please do not hesitate to give us a call.

HEALTH FUND:  Attention Medicare Part D Retirees: You may be receiving a phone call from Excellus (through the ELIZA Corporation) regarding your prescription drug coverage. This is a Coordination of Benefits (COB) Survey. This is information that the Centers for Medicare and Medicaid Services (CMS) requires so that member claims are processed correctly.

Your coverage will not be changed as a result of this survey. It only impacts which payer is the primary payer if you have other coverage. Please take the time to cooperate with this survey.

PERSONAL INFORMATION: If you have moved or have changed phone numbers or if you have changed your marital status, beneficiary status or dependant status please be sure to inform our office. We cannot help you if we cannot find you.

As always, please call us in the Fund office with any questions or concerns you may have.
